Information Collected
This public website collects no patient information. When you use the patient portal, the check-in tablet, or the clinic's staff systems, UCHC collects the information needed for your care, scheduling, communication and billing, such as your contact details, date of birth, health history, and any documents you choose to upload. This website does not collect or store credit card, debit card, or bank details; payments are handled by the front desk.
Secure Hosting
Patient information is hosted on Amazon Web Services (AWS) under a signed HIPAA business associate agreement. Staff sign in with a password and a one-time code sent to their email on any device that has not been verified. Access is limited by role, and activity in the system is logged.
Data Encryption
All connections between your browser and UCHCTX.COM are encrypted (HTTPS). Patient documents are encrypted when stored.
Data Minimization
UCHC collects only the information needed to provide and manage your care. Reminder and portal messages keep health details to a minimum.
User Account Security
Users are responsible for maintaining login confidentiality. Do not share passwords or sign-in codes.
No Absolute Guarantee
Reasonable safeguards are maintained, but no internet-based service can guarantee complete security.
HIPAA
When patient names and email addresses are connected to healthcare services or patient participation, that information may be considered protected health information under the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
UCHC maintains safeguards designed to support compliance with applicable HIPAA Privacy Rule and Security Rule requirements. Access to patient information is restricted to authorized individuals who require access for legitimate operational, administrative, technical-support, or healthcare-related purposes.
UCHC does not sell patient information or use patient information for unrelated advertising purposes.
